Advanced degree in pharmacy required. At least 3 years of experience in a hospital pharmacy preferred. Must have a current license in the state of Louisiana.
Interpersonal skills in order to communicate effectively with physicians and various other hospital personnel.
Category II Non Routine exposure.
RESPONSIBILITIES
1. Educates healthcare professionals and patients on safe and appropriate medication use including, but not limited to drug interactions and side effects.
2. Performs professional and technical activities associated with distribution of medications,
maintenance of drug inventory, patient records and patient financial records.
3. Supervises all activities of pharmacy interns and technicians. Adheres to policies, procedures and regulations to ensure compliance and patient safety.
4. Accepts total responsibility for orders including review, clarification, order entry, accuracy,
preparation, follow up and delivery to correct unit. Identifies, intervenes and assures resolution when problems involving drug therapy are identified.
5. Visually reads and interprets physician’s orders including drug product, strength, dosage,
start and stop time, initial inventory, quantity, etc. and assures that appropriate products are selected to prepare the dose accurately.
6. Routinely follows internal procedures in the pharmacy and on nursing units to verify
controlled substance inventories, minimize drug diversion and document wastes or missing doses.
7. Maintains clean and sterile environment within the clean room in accordance with USP 797. Routinely washes and applies appropriate germicidal solutions to all hoods, counters, carts, and all other equipment that will be used within the work area. Pharmacist is able to pass all requirements for working in the clean room, including gloved fingertip testing, media fill test, and written IV exam.
8. Reports any medication errors to appropriate clinical staff and documents via hospital reporting system.
9. Routinely checks for expired drugs in MedStations, Pharmacy department and floor stock.
10. Responds to Code Blue to provide assistance with medication selection, dosage, and preparation.
11. Performs nursing unit inspections, narcotics audits, and attends meetings involving the Pharmacy Department as directed by the Pharmacist in Charge.
12. Complies with facility Standards of Behavior and complete all required education assignments within the designated timeline.
